Include the app name and, if you know it, the version you're using.
For bugs: what you did, what you expected, and what happened instead.
Screenshots help a lot.

Privacy

Support emails are used only to help you and are deleted once the issue is resolved. See the privacy policy — the apps themselves collect no data at all.
